Corrected Sodium in Hyperglycemia

Calculates true serum sodium in the setting of hyperglycemia.

Parameters

mEq/L
mg/dL

Corrected Sodium in Hyperglycemia Result

--
FormulaCorrected Na = Measured Na + [1.6 × (Glucose - 100) / 100]

Clinical Pearls

Essential in the management of Diabetic Ketoacidosis (DKA) and Hyperosmolar Hyperglycemic State (HHS). As glucose is corrected with insulin and fluids, the measured sodium will rise to approach the corrected sodium value.

Limitations

Some literature (Hillier et al.) suggests a correction factor of 2.4 rather than 1.6 when glucose is > 400 mg/dL, though 1.6 remains the standard in most guidelines.
